1. Complete your ADHD Questionnaire.
   For chldren aged between 6-11 years, you will find the questionnaire by clicking HERE
   For children aged between 12-15 years, you will find the questionnaire by clicking HERE
   For children aged between 16-17 years, you will find the questionnaire by clicking HERE

2. Contact your GP Surgery and ask them for a copy of your GP Summary Sheet then email this into us.
